Analysis of Ethical Dilemma Essay

Natural calamities are unpredictable phenomena’s where the damage may be countless and immeasurable examples of natural calamities are earthquakes, floods and famine. In situations like this relief operations are challenging even though many organizations and nations extend their resources to overcome the disaster. We often face issues and concerns in a massive disaster which may lead to ethical-dilemma and criticism. When we analyze disaster situation ethical concerns arises. Here I would like to present the ethical dilemma involved in the rescue operations in Haiti disaster and analysis of the issues. Event and ethical implications One of the biggest and most recent disaster world encountered is the Haiti disaster in January, 12, 2010. It was a massive earthquake with 7.0 magnitudes, wiped out thousands of lives and affected millions of people (Haiti earthquake, 2010). The earthquake ploughed the main city Port-au-prince just into rubbles. Many people were trapped in the collapsed buildings. Many important buildings were damaged including parliament, schools, hospitals and main jail (Haiti earthquake, 2010). It resulted in lack of food, water, shelter and medical services. Many wounded were waiting for immediate medical help and many died without prompt treatment. In natural disasters people from all over the world offer humanitarian aid in the form of donations, medical care team services, food and other necessary supplies. The U.S Government and many other nations extended their hands for rescue operations. The Israel Government sent a military task force consisting of 230 people within 48 hours (NEJM, 2010). Their ultimate goal was to provide lifesaving treatments to the injured to the level it was possible. Other troops and volunteer workers on the scene also tried their best to deliver the best possible care to the causalities. Ethical concerns and ethical dilemma The troop’s mission was to provide help to as many people as possible. But the limited resources fell far short than the demands. This put the medical team in various ethical issues. Haiti is one of the poorest countries of the world with minimal facilities. The disaster left millions of people homeless and in urgent medical needs. The Israeli Field Hospital extended their hospital capacity to 72 from 60 and added one more operating room to accommodate increased causalities (NEJM, 2010). In a normal triage system we are setting priorities among patients depending on their clinical conditions and assuming that we will be able to provide care to all. In Haiti disaster it was impossible for them to provide care to all who needed. Here the health care workers as well as the victims faced ethical dilemma. The medical team was forced to recognize the fact that the clients who need most urgent care may consume the majority of their resources. So they had to set up priorities according to the resources available, the severity of the problem and can they be saved. Another ethical dilemma the medical team had to encounter was those come with severe injuries needed prolong rehabilitation and that may run out all of their supplies. So they planned to accommodate patients who can be stabilized in 24 hours.† The practical implication of this prioritization resulted in people those who have small chance to survive were not likely to be treated in the ICU† (NEJM, 2010). In addition to that discharging the patients were also challenging since no homes left and children without parents, there was no place for them to go. Personal ethical values and position using ethical principles In a massive disaster plan in an ethical aspect I believe the theory of Act- utilitarianism works best to that situation. Act-utilitarianism states, the actions people choose will produce over all good in any given circumstances. This theory also allows different actions in certain circumstances. Their decision on triaging the patients based on the resources and the possible outcome of the intervention in a humanitarian aspect. The ethical principles of beneficence and justice applied in this scenario. According to the principle of beneficence the health care workers were trying their best to provide care to as many as people possible. The ethical principle of distributive justice emphasis on fair and equitable distribution of goods and services (Burkhardt & Nathaniel, 2008), but in certain situations it is impossible for all people to have everything that they might need. In such cases they can formulate and enforce policies for fair and equitable distribution of the available resources. Possible alternatives for resolving the problem I.Ask for more medical supplies from organizations like, World Health Organization, Red Cross et. II.Make arrangements to send patients who need more complex care to the nearest available medical centers or hospitals. III.Call for more volunteers to help there by reducing the cost and request for funds from charitable organizations. IV.Since geographically U. S. is the closest country, request for airlifting the patient to the nearby states hospitals for advanced care. V.Even though insurance can be a hindrance to accept the patients, U. S. Government can offer tax exemptions and grants for the accepting hospitals. VI.Airlift the patients to the accepting countries for advanced care. Examine and categorize the alternatives When we categorize the alternatives from 1-6, the first five options are possible alternatives. The 6th option can be possible, but it will be more expensive, time consuming and unsafe for unstable patients. Since many organizations started their rescue operations, if the military task force can receive more supplies, they will be able to continue with their life saving measures. Possible consequences for the acceptable alternatives When other hospitals can accommodate more patients, it may reduce the burden on a single group. Since they already have disaster plan set up, more supplies from other organizations will help them to continue their task without time delay. With tax exemption and grants, more hospitals may take initiative to involve in the health care delivery and if the patient can be airlifted on time many lives can be saved. Prioritize acceptable alternatives I.Provision of more supplies to the functioning units. II.Make arrangements to transfer patients with complex issues to the nearest hospitals. III.Call for more volunteers for additional help IV.Airlift patients to the nearby countries at the earliest possible V.Request for more funds, grants, donations from the federal governments and common wealth countries. Plan of actions The plan of care is to provide care to as many people as possible with added resources. Transferring of patients to the nearest available hospital for complex care and management. Through financial aid rebuild the health care infrastructure to provide continued care to the Haiti people. Evaluation Natural disasters are unpredictable and every effort needs to be taken to overcome such situation through voluntary support. In this situation the ethics committee from a group of physician had to prioritize the cases according to the availability of resources and the chances of survival. In a humanitarian aspect it is necessary to provide fair and equitable distribution of health care as much as possible for a healthy recovery. We often see the best of humanity in moments of extreme calamity. In every disaster situations people who directly involved often faces some sort of ethical issues and dilemma. The best possible way to overcome such situations through mutual effort, ethical decision making and appropriate action plans. Fair and equitable distribution of health care is the best approach to every mankind.

A Double Standard in “A Doll’s House” Essay

In today’s society, all are taught the social structure that is meant to be followed and not changed; yet, are also taught to stand up for something if enough belief is put into it- a double standard by most accounts. Such standards exist in the play A Doll’s House by Henrik Ibsen. Set in the time where women’s equality was a joke and with an ending too shocking for the German public, this play brought into light the â€Å"two kinds of spiritual law one in man one in women† (Rosefeldt) this view outlines the distinct standards held for each gender that can be seen throughout different works of literature and even in today’s society. Rosefeldt emphasizes Ibsen’s view that this world is â€Å"exclusively a male society†¦with prosecutors and judges who judge women’s behavior from the male standpoint.† (Rosefeldt) From this, it can be agreed that such a standard exists. Looking back on literature, many unique examples can be found. In the epic, The Odyssey, Odysseus’s wife, Penelope, is remains faithful to her husband, of which has been gone for twenty years. Just in the act of being faithful lends to the fact that she was automatically expected to by her peers. No matter how much suffering, she was still to go with the social norm and wait. In stark contrast, Odysseus was presented with many chances of infidelity (the Sirens & Calypso) as the gods pressed obstacles in the way of his journey home. Despite the gods’ attempts, Odysseus’s own morals prevented the expected to occur. From this view, the story directly contradicts itself in regards to gender standards. Between the non-chalant attitude towards Odysseus if he were to cheat and the â€Å"Penelope must always stay faithful under and circumstance† perspective, it is implied that women are not at the same liberties as men when put in the same situation. In the play itself, the double standards set between Nora and her overly-dominant husband, Torvald, can be clearly seen. Many times throughout the play, Nora is referred to as a bird or squirrel as Torvald is always called by his name, implying that their relationship is â€Å"an overly-close bond† (Rosefeldt) – with Torvald in charge. As Nora struggles with her secret transaction throughout the play, she deeply ponders leaving Torvald and her family or even killing herself. Her deep consideration bluntly differs Torvald’s instantaneous decision to get rid of her as soon as he found out about her under-the-table loan. Greatly improved from the times this play was written, women aren’t property and most have their own jobs and careers. Even with their jobs though, women are still expected to be the homemaker. Throughout history, women have always been seen as the child-bearing counterparts to their bread-winning husbands-just look at any classic American sitcom. Although, much in those ways have changed in the past few decades, modern sports still hold the standard to a degree. Cheerleading, previously thought to be an exclusively girl sport, is increasingly growing popularity as a unisex sport. On the contrary, seldom is a girl seen playing football; rather rare to see a girl playing on a team other than in a pee-wee league or a backyard family game. In the end, whether agreed or not, there is, in fact, a double standard between men and women- though some subtle, these age-old and well-known lines aren’t and probably won’t be getting crossed anytime soon. Either way you look at it, one simple question sums it perfectly; would there be a story if it were Torvald who had borrowed the money?

She discusses many issues that are affecting the working class poor including health problems, lack of resources, lack of security and lack of adequate financial resources to live on. She continues to maintain that there should be a higher minimum wage and that one should be able to work for minimum wage and live on that amount of money. In order to do her research for this book, Ehrenreich agreed to live like the working poor. She worked at various places of employment including a small locally owned restaurant where she was a waitress; Merry Maids where she worked as a maid; and Wal-Mart where she worked as a store associate and all were negative experiences in her opinion. Capitalism is the idea that the economy of a given country should be run completely by the people of the country and that the government should not be involved in any way. Capitalism relies heavily on the idea that if the people were to control the wealth rather than the government than more human needs would be met. The people would also be in control of the ways of producing wealth and therefore wealth would be available to more in theory. Another principal idea in capitalism is the idea of land ownership. In capitalism those who own land would be able to sell their land to whomever they please or even sell the rights to use the land as an ongoing marketable property. The government would not produce fair market value for products either, the price of products would solely be based upon the market value which would be based upon supply and demand and a free price structure. I agree with Ehrenreich that there needs to be a lot of structural changes in our society. The only way to truly change the system is for everyone to be on board. I once heard a friend state that there should be a one house rule, meaning until everyone in the country has one house, no one should be allowed two. I feel strongly that the work that Ehrenreich did was important to those whom she was involved with as well as opening up the world of the working class poor to which many people have a hard time understanding. There were many things that I learned while reading the book Nickel and Dimed. I learned many things about what it would be like to be less fortunate than I am. There are many things that could happen to improve the system that involves us. The structural changes that were involved in the book included restructuring the minimum wage system, the way that the working class poor is able to afford basic human needs and that there needed to be changes to the health care for these people as well. Some structural changes that need to occur in order for there to be a better life for the working class poor including livable wages. It needs to be mandated and regulated by our government that the wages that someone earns are enough money to properly care for a parent and a child. Yes this could make it hard for a single parent with more than one child but for them to be at a wage where there was the ability to pay for themselves and one child would still be a vast improvement on what there is today. The wages need to allow a person to be able to provide housing, transportation, food and clothing. The problem with many low income families is that they get to a point where they have to choose between two of their needs. Take for instance in the book the young woman who was unable to afford to feed herself when she was pregnant as paying all of the bills was reliant upon her. She was working very hard as a pregnant woman for Merry Maids and working to ensure that there was a way to take care of an ill relative. This should not happen in our country where so many people are so fortunate. No one should have to go without food and set structural changes and better enforced regulations on pay would advance the quality of life for many who are a part of the working poor. Having wages that one can easily live on is very important. There are many things that could benefit the people of the working class poor if they were able to make enough money to survive on a month to month basis, rather than struggle. Some other structural changes that need to be made are issues with housing. It is sad to think of the fact that so many people in the US are homeless. The homeless in our country are not all unwilling to work. For instance just the other day I saw on Oprah a story about a middle class family with three children who were now homeless after being laid off and losing their home. Some of the structural changes that need to be made with housing are the way that people are able to find places to rent when they have little or no income through no fault of their own. I am not saying that there should be a way for people to just get free housing for everyone but rather that those who are in these situations and have worked hard and are simply laid off because of a bad economy are able to find a way to keep housing. The regulations made on housing and the way that housing is available to the working class poor needs to change as well. Another structural problem that needs to change is the lack of health care for those who fall in the working class poor. There is adequate health care through Medicaid if you don’t make money or if you only make little money. However someone who is working full time makes too much money to qualify for Medicaid alone. The person has to pay a co-pay in order to be able to get their Medicaid and usually they have a spend down to meet as well. A spend down is money that is out of pocket prior to any Medicaid benefits kicking in. The problem with this is that those who are working full time and earning only minimum wage are not making enough money to cover the cost of housing, food and their medical. Having a better and more readily available health care system for all is something that is important as many things that end up costing more lately could be prevented by having adequate regular medical exams and check ups. I am not even saying that there should be free national health care but rather that there are some ways that the availability of health care should be improved for all people. My suggestion would be a program that uses the social security numbers in order to offer free services to the American people. This would be like yearly physicals, generic prescriptions at reduced costs, yearly eye exams and reduced eye wear and yearly dental exams and reduced dental procedures. The charges should be based on a sliding scale fee with the person submitting income and monthly bill verifications. Another thing that could be done in order to make sure that employees are getting adequate health care is that the government could place regulations on companies about health care. The government could impose a percentage of an employee’s salary that a company could charge for health insurance and the government could limit the number of part time employees that companies have in comparison to how many full time employees have and if the company fell outside of those guidelines then they would have to offer health insurance to their part time employees as well. These are all simple things that would not cost the employers much more money and they are all things that could really benefit the entire working class poor in the long. Simple regulations have the ability to make the biggest changes in our economy. Another way that the government could regulate the medical insurance problems for the working class poor is to regulate the amount of money that can be employed for such health care services as a continuation in the direction that we are headed in currently is only going to raise insurance costs and make the availability of insurance to all even further out of reach. By having some set regulations for the cost of health care and the cost of health care insurance the government could easily help the working class poor to be able to better provide adequate health insurance for themselves. In the book Nickel and Dimed the author thoroughly explores the working class poor and their lifestyle. The author looks at the fact that without structural changes there will never be a better life for this entire class of people. These people will continue to struggle on a monthly basis, will continue to face homelessness, hunger and inadequate health care if there are never any changes made to the structural system that is indeed causing the problems for the worker. The system could make many simple changes that could greatly affect how these people, the working class poor, live their lives. These simple regulations and easy changes could make the world of difference to someone who is not as fortunate as others as this class of people would not have to go hungry again. Also it would greatly reduce the number of severely ill people who were unable to get early detection for major medical problems because of a lack of resources. Capitalism would allow the working class poor to be able to have control of more wealth and to control more of the terms of wealth that was available to them. It would give everyone an equal chance to be able to get some type of wealth system in place.
